All in for Health
Overview:
This project aimed to help All In For Health share public health information with the communities of Jackson and Josephine counties in Oregon in an easy-to-understand and engaging way. We created visuals to explain how the Community Health Assessment (CHA) and Improvement Plan (CHIP) were carried out in these counties.

Here’s the original MAPP 2.0, which outlines the steps for community health planning. We followed this structure but customized it to fit the specific process used for Jackson and Josephine counties.
Each of the main posters was paired with mini posters that included descriptions to provide additional context.
Phase One:
This phase focuses on the teams involved in the process. These groups formed the foundation of the community health improvement effort, ensuring strong collaboration from the start.
Phase Two:
This poster highlights how community input (through secondary data, surveys, focus groups, and key informants) shaped the health strategies.
Phase Three:
The final poster shows how the community will continue to build on its health improvements over time and showcases the three key priority areas for the CHIP.
Who Doesn’t Love Stickers?
To encourage participation in the CHA process, we developed a custom-designed sticker that fostered connection and celebrated participants' involvement after completing surveys or focus groups.
Design Approach:
Minimalist, hand-drawn style with mountains, trees, and a river to reflect Jackson & Josephine counties.Audience Insights:
Light research showed an audience preference for outdoorsy, natural imagery.Trust Building:
To address mistrust in government involvement, the hand-drawn look was chosen to create a friendlier, more approachable feel.
